PARENT/LEGAL GUARDIAN CONSENT & RELEASE

(Mandatory if Member/Participant is between 12 and 17 years of age.
Children under 12 are not permitted.)

I represent that I am the parent or legal guardian of the minor named
above. I have read and agree to all terms of this Agreement on behalf
of the minor. I hereby release and hold harmless HER PWR GYM LLC and
the Released Parties from any claims or liabilities arising out of the
minor's presence at or use of the facility.
